Welcome to Columbia City Sports and Spine!

  • Step 1 - Arrival

When you arrive we will ask you to fill out New Patient Intake Forms if you have not already done so online.

Please bring a copy of your Photo ID and Insurance Card.

Make sure to wear comfortable and flexible clothing.

  • Step 2 - Visit

Your visit will consist of a consultation with Dr. Nick, then his treatment and follow up plan.

During your visit please be prepared to talk about your medical history, the condition which brings you in for treatment and how that is impacting your daily life, any medications you are taking and any goals you have in relation to the treatment.

  • Step 3 - Treatment Evaluation

    This will include:

*Palpation: physical assessment of the tissues, bones and ligaments

    *Range of Motion Testing

*Orthopedic Testing: physical tests which will help us determine where the problem is

*Functional Assessment: includes body movements to assess overall functioning

  • Step 4 - Treatment

After performing an extensive evaluation, Dr. Nick will come up with a comprehensive treatment plan specifically for you.  Your first appointment will last 30-45 minutes long so be prepared to schedule that into your day.

  • Step 5 - Follow Up

Follow-up appointments are 15-30 minutes long pending on the diagnosis and treatment plan.

  • Step 6 - Between Appointments

Dr. Nick may provide you with therapeutic exercises along with movement and lifestyle advice. Please perform the exercises and follow the advice as given as it will aid in your treatment and lead toward more mobility, stability and less pain.

What to Expect After Your Appointment

Many patients experience significant symptom relief within a few weeks of starting their personalized treatment program. However, individual outcomes vary, and your active participation is crucial for success.  We provide ongoing care techniques and take-home exercises to support your healing journey. These tools empower you to strengthen your body at your own pace.

While most patients require minimal in-office follow-ups, we may recommend additional visits for those involved in sports, seeking proactive care, or dealing with more severe injuries. These follow-ups help ensure your healing process is progressing smoothly.
